Passages Case Management

Our case managers play a critical role in the
success of our programs. They provide guidance,
support, and encouragement for homeless
families and individuals. They are a source of
stability at a crucial juncture in client's lives.

Our Passages Case Management program was
started in 1997. This program provides a range of
services to all homeless adults and families in
Somerville, designed to move them from the
streets and shelters into permanent housing and
self-sufficiency. Every agency in the city that
works with homeless individuals and families refer
their clients to Passages, which then coordinates
all referrals for services and manages follow-up.

We work with over 200 clients a year.
